Community Connect - Individual Applications Public Questionnaire Form


Community Connect is designed to assist individuals and families who may be experiencing difficult life circumstances to address any struggles and live a life at their full potential. The questions asked in this application will only be used to determine program eligibility.

To meet the criteria for Community Connect an individual must meet the following criteria:
  • 18 years of age or older
  • Reside in North Dakota
  • Have a Mental Health or Substance Use Disorder diagnosis impacting functionality in multiple domains including housing, employment, parenting, physical health, and/or community connections.
